The health, safety and comfort of our guests and service providers is our #1 priority. We value your business and understand that mutual respect of time and communication will create a great experience every visit. Please initial as indicated and sign.
Spa Etiquette
_______Please arrive 10-15 minutes prior to your appointment to allot time for updating paperwork, using the restroom and/or enjoying refreshments. If you are more than 10 minutes late you may be asked to reschedule. If you choose to continue with a treatment after arriving late, your service duration may be shortened.
_______We are not able to accommodate additional guests or children who are not receiving a service.
_______Please limit cell phone usage to the reception area for brief calls to ensure a relaxing atmosphere for all.
Fees
_______A 75% reservation fee is required to book an appointment. Reservation fees and payments toward any package are non-refundable but may be transferred to another date if given 72 hours’ notice. Failure to reschedule at least 72 hours prior to your scheduled time will result in forfeiture of your reservation fee.
_______ A 10% hospitality and maintenance fee will be applied to all transactions.
Refund Policy
There will be no refunds on any products or services. Some products can be replaced if returned within 48 hours. This is determined by the 3rd party supplier. If you are not satisfied with your service, you must notify the service provider prior to leaving the spa. If we can make any modifications or adjustments, we will do our best to accommodate without interfering with the next guests’ appointment time up to 15 minutes.
Services
_______Please notify service providers of any allergies or conditions. Clients are responsible for pre and post treatment care. Please don’t hesitate to ask questions for clarity. We want to make sure that you have a thorough understanding and avoid any discomfort or adverse reactions. Results vary based on skin type, age, hormones, lifestyle, etc. It may take several visits to determine the best plan of action. Facial cupping, extractions, and massage services may cause temporary soreness and / or bruises. It is the responsibility of the client to indicate if pressure should be adjusted or request that the service provider discontinue.
_______Some services may be provided by a 3rd party contractor. All questions and concerns relating to the service must be directed to the contractor. Lemon Sage Day Spa, LLC will not be held liable for services or service agreements provided by a 3rd party contractor.
_______Services and outbound calls may be monitored by management for quality control or observed by apprentices/ students for training purposes. Concierges, apprentices, and/or students may enter the treatment room to assist the licensed professional providing your services.
SPA POLICY ACKNOWLEDGEMENT + AGREEMENT
By booking this service and/ or submitting a deposit I acknowledge and agree with the Lemon Sage Day Spa Spa Policy and booking agreement. I am releasing all persons representing this spa from all claims, demands, damages, actions and cause of action arising from the performance of selected service.
